Under LA root stumps with #15 & #36 region were extracted and immediate implant placement was done. Delayed placement was done in #45 & #46 region, soft tissue reconstruction surgery was done gain more keratinized peri-implant tissue around the implants by famous Palacci technique. Cover Screw was placed on #36 region implant and closure was achieved placing Geistlich Mucograft Seal and healing abutments were placed on #15, #45 & #46 region implants m aking it single stage surgery. Closure was achieved using 4.0 Cytoplast sutures. Implant placement was done free hand without using any stent or guide.

Clinical Follow-Up

Patient was asked to get Cold Packs for 24 hrs to reduce swelling. Medication was continued along with Tab. Flozen AA (Acelofinac + Paracetamol) twice daily & Chlorhexidine mouthwash 5 days. Patient is recalled after 7 days for Suture removal.

Conclusion

Immediate implant placement is always better option than delayed placement as augmentation procedures are prevented and Palacci Technique gives much better Peri-Implant Tissue. Single stage surgery gives much better patient compliance with a pre-requisite of high Initial Stability of 45+ ncm and High ISQ value of more than 60.

Under LA least Traumatic Flapless Surgery was done using Tissue Punch in #16 region & Implant Placed. Healing abutment was placed in #16 region Implant making it a Single Stage Surgery. Implant Placement was done Free hand without using any Stent or Guide conveniently as ample amount of Hard & Soft was present due to Ridge Preservation procedure which was done 24 weeks earlier.

Chosen Implant:

Position 16: Ø6.0/ 10mm – Dynamix

Clinical Follow-Up

Patient was asked to get Cold Packs for 24 hrs to reduce swelling. Medication was continued along with Tab. Flozen AA (Acelofinac + Paracetamol) twice daily & Chlorhexidine mouthwash for 5 days. Patient was recalled after 7 days for Follow-up.

Conclusion

Delayed Implant Placement procedure becomes a mandatory option than Immediate Placement to prevent incorrect Prosthetic positioning of Implant after Ridge Preservation Procedures when Implant placement & initial stability is compromised due hard & soft tissue deficiency. Case Selection & planning is very important for long term success & predictability. Single Stage Surgery gives much better patient compliance with a pre-requisite of high Initial Stability of 45+ ncm and High ISQ value of more than 60.
